COVID-19 Protocol

Dear Valued Patient,

We hope you and your families are coping during these difficult times. Since reopening on June 1rst, we continue to monitor and adapt our protocols to provide the safest environment. Eye Physician Associates would like to advise you of our updated protocols.

Please know that while some of these protocols may seem burdensome, they are for the protection of our patients, their caregivers, and our staff.

Please read through some additional updates below and call our offices at 414-928-2020 with any questions.

If you had a visit that was cancelled due to COVID-19, we have attempted to reach you. If we missed you, please call as soon as possible for advice on when to arrange follow-up care.

Some general protocols for all offices are listed below. They were designed to ensure social distancing, shorten wait times, and ensure everyone’s safety:

  • All patients and visitors must wear masks in the office. While we encourage you to bring your own mask, those without masks will be provided one.
  • Each patient may have up to one visitor accompany them into the office. The visitor cannot wait in the in waiting room.
  • We ask that only caregivers join patients in the exam room. A caregiver is defined as someone who assists a patient with movement, language translation, or with understanding diagnosis and treatment plans.
  • Given the more confined space due to his extra staff and supplies. Dr. Mukhopadhyay kindly requests that caregivers are not in the exam room He is happy to call anyone during and/or after your visit.
  • Part of our screening process will assess adherence to social distance recommendations. If you have not been practicing social distancing you may be asked to reschedule. If you have questions about this, please call.
  • We will ensure that patients with emergencies are seen promptly and safely

Some protocols vary by office and all are designed to adhere to safety guidelines:

Mequon and Oak Creek Offices

Please wait in your car prior to your exam. When you arrive, please TEXT the number provided in the appointment reminder you are sent. Our front desk will call you and screen you over the phone.

St. Lukes and Airport Office

Upon entering the building you will be screened by Aurora employees who will also provide you a surgical mask if you do not have one. These employees will also give you a screening ticket which we ask you to keep. We will ask additional screening questions when you enter our office.
